Surgery catchment area

You may register with the surgery if you live within a radius of 5 miles.

At present we are not accepting patients who are already registered with other practices in the local area due to current pressures on our services

Banbridge Group Surgery comprises 3 GP practices:

Patients wishing to register with the surgery must indicate which practice they want to join.

If you have been registered with and attended an NHS GP surgery within England, Scotland, Wales or Northern Ireland in the past year or you have returned from the armed forces, Please download the following form

If you have NOT been registered with or attended an NHS GP Surgery within the UK in the past year, please download the following form:

NB:

  • the form must be completed in full and signed and dated
  • the completed form must be returned to the surgery
  • patients completing form HSC R1 must produce relevant documentation as outlined on the form
  • non EU nationals must produce their visa
  • write the name of the practice with which you wish to register on the top of the form

Because it may be some time before we receive your previous medical notes, it is important that each new patient registering completes a health questionnaire.  This should be returned to the surgery with your registration form:

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
